Request a Repeat Referral

This service is for repeat referrals only. You will need an appointment for any new referrals.

Mental health referrals will always require an appointment.

Request a Repeat Prescription

We are now able to send electronic prescriptions directly to your email. Please include your current email address when requesting prescriptions. This service is for non urgent requests only and for patients who have attended the practice within the previous 6 months. Please allow 3-5 business days to be actioned.
